New Client Information
For more information about my practice and rates, please read my Office Policies and Procedures. Please read this first before attempting to schedule an appointment, as it will answer many of your questions.
If you want to set up a first time appointment with me, please go to the Client Login page and register. Once all the required forms are completed, you will be able to go to your own personal and secure client portal. Look for the calendar icon on the top menu bar and choose an appointment date and time. I will be notified of your request, and will get back with you, in most cases, within 24 hours to confirm, or work out another date/time with you.
Please note, until the required forms have been completed, your registration will not be completed, and you will be unable to access any of your portal features, or make an appointment.
