Even if your insurance is not listed below, check with your employer to see if you are eligible for Employee Assistance Program (EAP) sessions with me.
Before your first visit: Please call the number on back of your insurance card to find out if your coverage provides for mental health benefits, whether they have any pre-authorization* requirements, what your co-pay will be, and if there is a deductible you must satisfy.
*If you must have authorization in place before you see me, be sure to ask them for the authorization number, how many visits this authorization covers, what date the authorization expires, and to what address should the claim be mailed. Also, please ask for their name and number so our office can call to follow-up with them. You will need to call or email me with this information before your first visit, as well as bring it with you to your appointment.
